When billing for Newborn infant using mother's ID ( hospital billing)

1. Please ensure that BOX 4 contains the mother's name, not the child's name.

2. Box 6 relationship to insure is should be place as Child

   

Once claim is recieved turnaround time for a response is 45 days.

Once a claim is denied we have 1 year to submitted an appeal or a corrected claim.
